Eligibility Condition for Signature Card

LATEST POLICY STATUS REPORT/PREMIUM RECIEPT/PREMIUM PAIDCERTIFICATE OF INFORCE POLICY IS REQUIRED

Salary Slip if Submitted must incorporate Bank A/c No., Bank Name & Company logo if not then 3 Months’ Bank Statement is required (Not required if Applicant has account in Axis Bank)
